Conditions of membership

In submitting an application for membership, you acknowledge the following:

• I declare the information provided is correct.
• I agree to accept responsibility for any items borrowed on my card. I will not lend my card to anyone else.
• I agree to pay any charges that may be incurred through the late return, damage or loss of an item.
• If unpaid invoices are referred to debt collection, I accept responsibility for payment of the associated costs.
• I undertake to inform the library of any change of address or contact details.
• I will contact the library if my card is lost or stolen, otherwise I am responsible for items borrowed before it is reported.
• I authorise Waimakariri Libraries to make available the information about me contained in the membership form for the purpose of debt collection.
• If acting as a guarantor for a child under the age of 16: I agree to accept responsibility for the supervision of my child, their library card, all items borrowed, any charges or fees incurred, and the content of materials chosen and/or viewed.

Statement for the purpose of Principle 3 of the Privacy Act 1993

• The information about you is being collected by Waimakariri Libraries, a unit of the  Waimakariri District Council. This information is not required by law. However, it is necessary if you wish to become a member of the library and borrow material.
• This information will be held by Waimakariri Libraries, PO Box 386, Rangiora 7440.
• You have the right to see personal information about you held by the library and to ask for it to be changed.
• Your personal information will not be shared with anyone else except where this is necessary for debt collection on library material or required by law.
